Dear Sir or Madam:
I would like to apply for the Andover/YMCA Community Center Advisory Commission
with the city of Andover: I believe the Community Center and its connection with the
YMCA provides an important family friendly center that can pull together a range of
members from the community. I would be an asset to the commission due to my
professional and academic experiences.
My business, Jaworski Law, is located in Andover and focuses on estate planning and
end of life planning. I have participated at the YMCA Business Partners Expo and Silent
Auction and truly believe that local businesses are what keep a community thriving.
Routinely in my law practice I must negotiate conflicting interests and adhere to legal and
ethical regulations.
While at ElderCare Rights Alliance I coordinated stakeholder meetings and was part of a
legislative reform effort from its inception. I developed working relationships with a
range of stakeholders and provided legislative research and technical support. My work
with Historicus, Inc. included intensive historical research, as well as budget and project
proposal drafting. I assisted in the production, writing, and copyright management of
twelve documentaries published on DVD. I coordinated company interaction with
outside contractors and negotiated scheduling conflicts with talent, and contract rates for
copyright licenses.
Outside of my professional experience, I have spoken in front of many different groups
including a presentation of my research at the Canadian Elder Law Conference in
November 2008 in front of internationally recognized elder law professionals. I am also
an active member in many legal organizations and an active YMCA member.

I have served on the Commission for approximately 6 years however most know I have been
involved for approximately years as an original participant Team member making the CC asset
a reality. My focus is balancing fiscal responsibility and while servicing Andover residents To
date, the facility is not far off facility projections, considering fundraising shortcomings and the
current economy. Business development is critical to continued success of the facility. I have
promoted the hiring of a business development person two years ago and today Cindy will
dedicate her complete time on the job focusing on BD. I wish to continue my role within the
Commission at least through realization of a cash flow positive position and deliver to the
residents of Andover what the Council and I promised them during our public meetings Our
original vision for the CC was public and private collaboration and I intend on continuing to
promote this mantra for the City of Andover residents in the spirit of minimizing tax burden to
all.

Economics Degree from the University of Michigan however as importantly is my 25 years of
experience as an Entrepreneur managing a global environmental remediation business
successfully.

See above... and I have served the City and it residents reviewing GW analysis for the WDE
landfill, met with USEPA and convinced them to install a flameless flare at WDE and to locate
the flare on Crosstown, not on Hanson Blvd. Furthermore I served as a resident (along with
CCM Trude) in strategizing and communicating with the AH school board to promote locating
the new HS in Andover and not Rogers. Organized support from colleagues from the AH school
board and also Anoka County Board. Appointed by the CC as the Chair of the City of Andover
Sports Complex Task Force and lead a Team in which gained momentum and achieved realizing
5 votes from the existing CCM. Todays CACCAC is a continuation of a 9 year effort in leading
participants to the reality of one of todays most prized assets within the City.

Mr. Kellogg is a key component to the continued success of DCI
Environmental, Inc. (DCI). He joined DCI in 1995 in a merger of a
company he founded in 1986. He is responsible for financial, technical,
operational, business development, and business affairs. He brings to
this TEAM a superior and dynamic understanding of business as it
relates to the environmental field. His asset to the environmental
industry is his ability to formulate and encourage out of the box thinking,
as well as the ability to think inside the box, when necessary. His overall
grasp of inner relations of doing business in the environmental industry
has enabled DCI to be a forerunner in the alternative technology
remediation industry in accordance to USEPA domestically. In 2009 Mr.
Kellogg assembled a Team in which organized to win one of USEPA's
Mega -Site Superfund Sites identified within the United States. This
Mega -Site Superfund site's RI /FS remedy fiscal spend is estimated at
$85 MM to $95 MM. Also in 2009, Mr. Kellogg serves as an advisor to
the Wyckoff Creosote Superfund and the St. Maries Idaho Superfund
site(s) and continues to do so at the request of the Washington
Department of Ecology (Ecology) and USEPA. Internationally, he has
led DCls business development in which consummated project awards
within the Czech Republic and Norway. In 2009, Mr. Kellogg's business
development efforts lead to a short list (5) accomplishment within the
Kingdom of Saudi Arabia regarding the KJO AL- KHAFJI Joint
Operations (Amamco and Kuwait Oil Company) in which received 93
prequalification (PQs) applicants from around the world. Furthermore,
the Kuwat Oil Company (KOC) has requested PQ submittals for
remedial assistance regarding the post 1991 invasion oil lakes
contamination in which he organized and is awaiting short list notification
due by end of 2011. He has over 25 years of administration,
managerial, and operations experience ranging from large -scale
production facilities to finance organizations.
Mr. Kellogg provides a full range of financial and environmental
consulting services in the areas of environmental affairs; assessment,
remediation and redevelopment of contaminated sites; soils remediation
feasibility studies and selection; cost estimating and management;
treatment of contaminated soils, sediments .and water; and,
environmental management, including 10 -K SEC support.
Mr. Kellogg has served as an external member of the board of
directors of privately held financial and environmental companies. He
has also served on an advisory committee for the Department of
Defense (DOD) during the Base Realignment and Closure (BRAC)
activities and the United States Environmental Protection Agency
(USEPA), on some of which he continues to serve.
Mr. Kellogg has managed administratively and operationally (including
Program and Project Management) approximately 105 contaminated sites
across the county consisting of petroleum hydrocarbons, chlorinated
solvents, PCBs, herbicides and pesticides, dioxins, wood treating creosote,
and Former Manufacture Gas Plant (FMGP) coal tar and carbureted water
gas tar waste. During his oversight with DCI, DCI has successfully
completed projects for Federal Agencies (Air Force, Army, Navy, and
USEPA), large transportation companies (BN &SF, Boeing, C &NW, Soo
Line), major oil companies (Amoco, Texaco, Con neco /Phillips, and Mobil
Oil), and utility companies (MidAmerican Energy Company, Alliant Energy
(Wisconsin Power & Light, Iowa Electric Services, and Interstate Power
Company), Northwest West Public Service, Center Point Energy -
Minnegasco, We Energies (formerly - Wisconsin Electric Power Company
and Wisconsin Gas Company), Wisconsin Public Service, XCEL Energy,
and Kansas Gas Company.
For the last fifteen years, he has Program, Project, and Co-
Project Managed 83 FMGP, wood treating and herbicide and pesticide
sites during RI /FS, design and implementation of civil construction and
remedial activities and has remediated over 2 million tons of FMGP /PAH
contaminated media for various Utility and wood treating clients directly.
He has participated in deliberations with the USEPA Regions 5, 6, 7,
and 8 and various State Agencies in which performed the Regulatory
oversight of the Utility FMGP and wood treating projects. Technologies
in which his expertise has managed are Low Temperature Thermal
Desorption (LTTD), Medium Temperature Thermal Desorption (MTTD),
Incineration, In -situ and Ex -situ Chemical Enhanced Soils Stabilization
(SS), and In -situ Chemical Oxidation (ISCO). Furthermore, he has
designed and built a Solid Waste Treatment Facility (SWTF) utilizing
methane gas as an alternative fuel source of energy to reduce the costs
of goods sold (COGS) within the operations and create sustainable
energy concepts and reduce DCls carbon footprint. This concept was
recognized by USEPA Headquarters and also allowed a major utility to
realize a substantial Federal tax credit within its electrical generating
facility.

As founder and president, Mr. Kellogg's delegation of authority
allowed key employee's to perform at their optimum potential while at
the same time giving him the opportunity to review and oversee from a
management point of view. This style allowed him to meet with
government regulators, existing and potential clients, as well as oversee
and manage the financial aspects of the company. Mr. Kellogg's
primary business focus was working with the DODs base closure
remedial activities. This business development leadership
consummated direct contracts with DODs Air Mobility Command (AMC)
Headquarters, the Air Force Center for Environmental Excellence
(AFCEE) and Bechtel
Mr. Kellogg's insight into government regulations and continued
monitoring of the ever - changing regulations made him the primary
corporate liaison between AST and the multitude of regulatory agencies.
His business and environmental remediation intuition led to Mr. Kellogg's
seat on the Contaminated Sites Technology Review Board for the
USEPA Region V and VII (one of 4 seats on the committee and the only
private sector seat). This board was created to review technical and
implementation stages of the alternative technology manuals that are
used by both State and Federal regulators today. A subsequent
organization has formed and is known as the ITRC Council.

As you can see from my enclosed resume, I have a unique educational and career background
that would be an excellent fit for the position described in your ad. Not only do I have an in-

very excited to find the posting of the position as I have been looking for an opportunity to get
more involved in the Andover community and feel this fits my experience and passion.
In 2002 I completed a Bachelor of Science degree from the University of Wisconsin -La Crosse
with a major in Recreation Management. At that time I began my career in the field of recreation
and soon started to work for the YMCA in Rochester, Minnesota. There I started as the Aquatic
Director, where I managed the lifeguards, swim instructors, and swim team for a period of two
years. I was then promoted to the position of Senior Program Director, where I supervised the
Youth and Family Director, the Health and Wellness Director, the Preschool Director, the Facility
Manager Coordinator, and the Aquatic Director. In this position, I was also responsible for our
Day Camps, Youth in Government, Program Committee, and the budget management of
5800,000.
In the spring of 2011, my husband accepted a position in the Twin Cities and we relocated our
family to the city of Andover. I am currently working part-time for the Robbinsdale Area
Schools in their Community Education department and raising my two young boys. I am looking
to stay current in the field of recreation and enjoy volunteering in the community. I feel I would
be able to bring my background to the council and a fresh perspective to the issues.

I enjoy hiking and biking through various natural areas and have a strong appreciation for the
wild animals and plants outdoors.

Unfortunately, at this time in my life, I find less time than I would like to pursue my
interests outside of work and home. In the past I have been involved in organized trail clearings
at the Coon Rapids Dam, I have been a volunteer worker at a YMCA wilderness camp, and have
been frequenting the Boundary Waters canoe area as often as possible for the last 20+ years. I am
hoping that by serving on this commission, I can give back to the community, in an area that I am
passionate about, despite my limited free time

When Andover was just becoming a city, I served on both the Planning Commission and the City Council,
helping to begin the process to make Andover a high - quality, forward thinking community.
I professed a position of support for the conservation of our land by participating in drafting the first
Comprehensive Plan which the City produced. Embodied in that document was a position statement on
preserving open space. I testified before a Minnesota Senate Committee on Land Use Planning. All of
my positions on land use were toward controlled growth, thereby using our developable land in an
efficient manner and conserving land areas for future generations. It is gratifying to see that Andover
has an organized advisory group of persons singularly devoted to the procurement and management of
open space.
I assisted in drafting and interpretation of ordinances, and was involved in the administration and
enforcement of all of the ordinances in the City of Andover. Likewise, the implementation of the
Comprehensive Plan and establishment of a Capital Improvements Plan were duties of the City Council
during my service. I served on Andover's Historical Committee, and the Personnel Committee, and spent
many hours on other duties of the Council.
I have no conflicts of interest within Andover: I am a respected business woman in Anoka and although
have many acquaintances among the City Officials, I am not involved in activities of the Anoka City
Government. I own a home on a 2 -1/2 acre lot, and own no other land in Andover. I am not involved in
developing, selling or procuring any real estate in Andover.
I served a three year term on the Anoka Area Chamber of Commerce but no longer hold that position.
My service on the Anoka Rivertown Community Partnership was for 1 -1/2 years and along with my
service on the AACC, I gained additional experience in reviewing growth and services to the citizenry,
procuring funds, and planning for the future within the parameters of an organization dedicated to the
betterment of the welfare of a municipality.
I am interested in continuing to participate in the growth of the City of Andover. Certainly my years of
experience would be put to good use by my service on t-he Open Space Advisory Commission.

I relocated from Duluth in 2006 and have been involved with youth sports in the area. I
am the club director of the largest volleyball club in the area, working with the staff at
ACC to run a 14 team volleyball club that supports over 120 athletes each season.
I am the league director of adult hockey and volleyball leagues in Duluth, renting space
and directing all aspects of the leagues, so I have a great perspective of fees and costs to

I have served on the Andover Planning & Zoning Commission since 2000 and as Vice Chair since
approximately 2006. 1 feel I bring continuity and history to the Commission. This is especially beneficial
during the current economic slowdown since the Commission's agendas are fairly light but equally as
important as in days past. I enjoy serving my community.

Community and Regional Planning (NDSU). I have worked for Anoka County, managing their transit

I am a current member of the commission, and I felt I have service the commission well. As
there always are, there are a number of on -going issues on which I have been part of the
discussion and desire to continue to work towards resolution on these issues. I feel the park
system is an inortant part of our city and special attention needs to be paid to our parks and the
opportunities provided as our city evolves in age, demographics, etc.

lived in a number of cities throughout the mid -west and have been involved in many activities in
the communities in which I have lived — adult sports leagues, numerous youth sports leagues
including starting the youth wrestling program in Andover, scout leader, school advisor groups,
etc.
I feel this diverse experience puts me in a good position to consider a wide range of perspectives
when considering issues.
I have a BA in Business Management, a minor in Sociology, and an MBA. I am also a FINRA
