1. Call to Order - 7:00
Attendance: Mike Knight, Dan Terhaar, Devon Walton, Todd Schoff elman, Frank Kellogg,
Traci Warnberg-Lemm, Erick Sutherland.
2. Approval of Minutes - Monday May 9, 2005.
The May 9th minutes were approved, with the following modifications.
4B - Add the following to the end of the paragraph. In addition further discussion will be
needed in regards to age bracket, fee structure, and times of day.
4C - Rewritten: The commission recommended approval of a one year contract for non
prime to Andover High School, for field house use. The hours are 3:00-5:00pm from mid-
September to mid-May at a rate of $30.00 per court hour. This was a negotiated rate for the
high school. Prime rates are $35 per court hour from 5-lOpm during the week. Discussion
centered on only approving two courts at this time for the high school. The commission
would like the remaining court to be left open for open gym programs.

3. Discuss Field House Usage
A. Walking Program
The Commission discussed the pros and cons of a fee for service walking program.
Overall the Commission was supportive of not charging for walking and jogging, but
wanted staff to research the potential of business sponsorship. The commission has asked
for staff to develop a spread sheet, showing what other facilities are doing in regards to
fees, staff, and schedule when it comes to the walking program.
B. Rental Proposals
The commission was updated on the letter that was sent out to possible users of the field
house. The requests are due back no later then June 3rd, 2005. The commission will have
a better feel for rental proposals once the requests come in. The commission discussed
the idea ofleaving one court open at all times to facilitate open gym times throughout the
evening, weekends.
4. Advisory Commission Open Discussion
A. The Commission indicated they would like to continue discussions on the
concessions at the next meeting. The commission has asked staff to look
into the grilUfryer. Costs and/or a partnership to use a grill/fryer in return for
buying product.
B. The commission has a list of things they have asked staff to get answers on
prior to the next meeting. They include staffing, continued discussion on the
field house when it comes to fees, open gym schedules, and hourslrates.
C. The commission has asked for a break down when it comes to expected
revenue per space that the city will operate. TIlls information will allow the
commission to better understand what types of programs need to be run to
achieve our revenue goals.
D. A map of inside and outside the building is requested to break down the
operation of the facility and who will oversee all areas. In specific the
scheduling of the Community Rooms and the cleaning of all areas inside
and out.
5. Other Business
There was none.
6. The next two meeting were scheduled.

Attached for the Commission reference are a summary of survey data and some of the actual
correspondence relative to gymnasium usage. The commission will be asked to discuss the
following items relative to the field house:
1. Walking Program Update - Staff has placed calls to some local businesses and
local clinics to see if sponsorship of a walking program is desirable. Staff
looks at the sponsorship similar to the current advertising in the ice arena.
Staffwill provide a further update to the Commission at the meeting.
2. Rental Proposals Update - Mr. Sutherland sent out letters to potential users to
survey interest in field house rentals. Staff will be meeting with interested
parties on May 23rd. Further progress will be discussed with the Commission
at the meeting.
3. Rental Guidelines - Staff has sent out some e-mails to other facilities to gather
guidelines being used at those facilities. Staff will present what is received by
Monday.

Friday, May 27th will be the last night for open gym until
next fall. Open Gym will resume on Friday, September 9th
from 7pm-llpm.
Ralia hosts Friday Night Adult Open Volleyball. The four
courts will be available from 7 pm to midnight. The cost is $5
per player.
All participants must have a signed waiver on file. If you

NOTICE: Requirements to use the
Maple Grove Community Center Gym
GOAL
The Maple Grove Community Center has experienced a continuous increase in use of the gymnasium and rising
concerns surrounding the gym operation. An evaluation of the use patterns, open gym schedule, rules and policies,
security matters and other issues relating to the gym was completed and patrons will notice four main adjustments to
the current services. The adjustments made to the gym operation support our goal to provide patrons of all ages a
friendly, fun and safe gymnasium. It is important to be aware of the following changes if you plan on using the Maple
Grove Community Center gym.
KEY HIGHLIGHTS
I. All patrons must register to use the gym and have their photo entered into the I D system.
2. The gym schedule has more defined open gym times specifically listed for family, individuals, tots, teens, adults,
residents, non-residents & annual pass holders.
3. A Zero Tolerance Policy has been implemented and is enforced.
4. Gym admission and registration will be accepted at the gym or pool customer service desks.
RESIDENCY
Residency as referred to on the Qvm schedule is defined as people who live in Maple Grove, have full-time
employment (40 hours weekly) in Maple Grove, or is attending Maple Grove/Osseo Junior or Senior High School.
REGISTRATION
WHO NEEDS TO REGISTER? Everyone that is age nine (9) and older must register.
WHO DOES NOT NEED TO REGISTER?
Groups or individuals that are exempt from registering include recreation program participants,
daycare's, Birthday Party Package customers, groups or private rentals scheduled through the
Rental Coordinator, All Building Annual Pass Holders, youth/tots under nine (9) years of age or individuals as
determined by Maple Grove Community Center management.
WHAT IS THE A FEE TO REGISTER?
The gym registration is free of charge. Each patron is required to fill out a registration form and have a digital
picture taken that will be used to confirm a person's identity and residency each visit. Youth ages nine and above
are required to sign the form as a confirmation they understand the rules and will follow the rules.
WHERE DO I REGISTER?
Registration will take place in the gymnasium customer service desk or pool customer service desk.

WHAT TO BRING TO REGISTER FOR MGCC GYM
Be prepared. Bring the necessary information to complete the registration.
. Provide accurate information of a patron's name, address, current home telephone number, and date of birth.
Falsifying information is an automatic expulsion from MGCC property indefinitely.
. Youth age 17 and under are reauired to have a parent present to sian the reaistration form and provide a proof of
residencv with each youth reaisterina for the aym. unless the youth is aae 16 or 17 an has their own driver's
license. The gym registration form will not be allowed to be sent home for signatures or to have the parent come
at a later date to sign the form.
. Residents and non-residents of all ages are required to present an acceptable form of ID.
Acceptable Forms of ID to reQister below. All ID's must have a photo and an address-on the ID.
. Drivers License
. Non-Residents working full-time (40 hours weekly) in Maple Grove are to bring a currEffit pay stub and drivers
license
. Minnesota Identification card with photo and address (for ages 21 & younger)
. MGCC Resident ID
. State ID and utility billing in the patron's name
EACH VISIT & ADMISSIONS
Patrons must provide their home telephone number for MGCC staff to look up the photo I D and registration
information. Patrons will then pay the resident or non-resident admission fee. Annual All Building Pass Holders will
scan their ID's at the gym desk or pool desk. All patrons will receive a wristband.

SCHEDULES
Monthly gym schedules are available in racks near the customer service desks or online by going to
www.ci.maple-arove.mn.us. Gym closures, group visits, holiday hours and other activities scheduled in the gym are
updated for each month and listed on the schedules. Changes are posted at the service desks and/or at the front
entrance.
NEW RULES
A ZERO TOLERANCE POLICY and additional gym rules were implemented to ensure a safe, fun environment is
available to all gym users. Patrons are responsible to know and adhere to the rules.

Meeting Room Rental
ROOID_RentaLReQuest Form (PDF) - print form and send by fax, mail
or drop off in person at the Maple Grove Community Center.
For more specific information or to make a rental request, please call
the Maple Grove Community Center pre-recorded information line at
(763) 494-5969. Leave your name, address, phone number and
preferred dates and times.
NOTICE: The rental coordinator works Monday - Friday from 8:00 a.m.
- 4:30 p.m., please leave a number where you can be reached during
these hours.
RENTAL HOURS:
MONDAY - FRIDAY 7:00 a.m. to 10:00 p.m.
SATURDAY 8:00 a.m. - 10:00 p.m.
SUNDAY 12:30 p.m. to 9:00 p.m.

Directions to the Community Center
Features:
The room sizes vary to accommodate 15-40 people depending on room
set up. Portable walls provide flexibility for accommodating large
group functions. Rooms are scheduled on an hourly basis.
Table Set-Up:
The room will be set up with 6 foot tables in a square, theatre (rows)
or "U" which is confirmed upon scheduling your room rental.
Decorations:
No tape or adhesives on walls or furnishings during your meeting room
use. We will provide hooks, magnets or other accessories to assure
that no damage occurs.
Food/Beverage:
Bring your own food or schedule our food service upon scheduling the
room. Select from coffee, pop, juice, muffins and cookies.
